Remote Patient Monitoring and Chronic Care Management are Medicare Part B services. That means Medicare covers 80% of the cost, just like an office visit or lab test. The remaining 20% is usually taken care of by your secondary or supplemental insurance. To check for any co-pays, just reach out to your insurance provider and ask about CPT codes #99494 and #99490.

No, you can always share your health updates over the phone. However, if a family member or friend has an email they’re willing to use, that can be helpful for receiving program updates. You can also check email at places like the library, church, or senior center.

Absolutely! You can call during business hours or send a message through our Nsight app. Plus, you can log any health updates between calls, so when you do chat with your coach, they’ll have all your latest info at hand. Access to the app is available as soon as you sign up.

There’s no set rule on how often you log updates—just do what works for you! The more information your doctor and Health Coach have, the better they can support you. We want this to feel helpful, not like a chore. Each practice reviews logs differently, and in addition to your doctor, their care team (including nurses and medical assistants) may also check your updates.

No problem! A caregiver can communicate with our healthcare team on the patient’s behalf, as long as we have the patient’s approval. Just provide the caregiver’s name and contact info so we can keep them in the loop when needed. The caregiver can also access the patient’s Nsight account using their own email, with permission.
